The Person-Centered Plan (PCP) is the foundation of everything we do. It ensures that you are the expert and the primary decision-maker in your own life. Our specialist do not impose pre-made solutions; instead, we sit down with you to identify your unique strengths, specific goals, and personal aspirations. This collaborative planning process results in a highly customized Individual Program Plan (IPP) that is completely tailored to your needs and requirements. This focus guarantees that our support—whether for skills training or housing—is relevant, empowering, and directly contributes to your chosen path toward independence.

Exploring personal interests, skills, and career goals through individualized planning and goal-setting
Supporting enrollment in educational programs, vocational training, adult education, and certification courses
Assisting with job readiness skills, including résumé development, interview preparation, and workplace communication
Teaching clients how to search for jobs, complete applications, and follow up with employers
Providing guidance on accessing community resources such as workforce development centers, community colleges, and employment support programs
Supporting clients in understanding workplace expectations, schedules, and professional behavior
Encouraging self-advocacy in requesting accommodations and supports when needed
